Increasing Prevalence of Chronic Diseases
Rising prevalence of chronic and lifestyle diseases in China is a core structural driver for wearable medical devices because it creates a large population needing continuous, low‑burden monitoring rather than episodic, hospital‑based care. China’s chronic disease burden already exceeds 300 million cardiovascular patients alone, including an estimated 245 million people with hypertension and nearly 130 million adults with diabetes, illustrating the scale of long‑term management needs. National surveys report hypertension prevalence around 23–28% in adults and diabetes prevalence above 11%, trends that have risen over recent decades in parallel with urbanization, sedentary lifestyles, and dietary shifts. These conditions require regular tracking of blood pressure, blood glucose, heart rhythm, and physical activity, which wrist‑worn monitors, smartwatches with BP/ECG, and smart patches can provide at home and in community settings.
Wearables capture real‑time data, enabling early detection of deterioration, reducing avoidable hospital visits, and supporting long‑term prevention strategies that Chinese public‑health authorities emphasize. For example, hypertension affects hundreds of millions of Chinese adults but blood‑pressure control remains low, so BP‑enabled smartwatches and connected cuffs linked to mobile apps and teleconsultation platforms are being used to remind patients to take measurements, flag out‑of‑range readings, and support medication titration.
Similarly, diabetes prevalence above one in ten adults has spurred interest in continuous glucose monitoring sensors and data‑sharing apps that let endocrinologists review glucose trends remotely and adjust therapy. Policy and infrastructure reinforce this pull: initiatives such as “Internet Plus Healthcare” and the 14th Five‑Year Plan for health informatization explicitly promote digital tools, remote monitoring, and health‑data integration, while over 4 million 5G base stations deployed by late 2024 make streaming of wearable vitals practical across most urban areas.
Technological advancement
Technological advancements propel the China wearable medical device market by enabling more accurate, real-time monitoring and therapeutic delivery through innovations in sensors, AI, 5G connectivity, and flexible materials, addressing limitations of traditional episodic diagnostics. Miniaturized Oscillo metric cuffs in devices like Huawei's Watch D2, launched in late 2024, provide cuff-like ambulatory blood pressure tracking over 24 hours alongside ECG, SpO2, heart rate, and sleep analysis, earning NMPA medical device certification and insurance reimbursable in China, unlike optical approximations in competitors. This shift supports continuous vital sign surveillance for hypertension patients, integrating with apps for physician alerts.
Flexible hydrogel-based multimodal patches detect proximity, pressure, and temperature with 0.5 °C⁻¹ sensitivity, conforming to skin for non-invasive respiration, pulse, and wound monitoring in chronic care. AI algorithms process multi-sensor data for predictive diagnostics, such as early cognitive impairment detection via wearables, as explored in global collaborations now localizing in China. With over 1.1 billion global 5G subscribers by early 2023 enabling low-latency streaming, China's 4+ million base stations facilitate cloud analytics for remote interventions in smart wards.
Electronic skin patches embed wireless sensors for real-time physiological data, linking to telemedicine for anomaly detection in daily life. "Made in China 2025" policies integrate AI models like Baidu's Ernie into smart rings and glasses, fostering domestic ecosystems with tech giants. Pulse-mapping sensors blending traditional Chinese medicine with ionogel arrays visualize arterial waveforms in 3D, simulating clinical palpation for precise cardiovascular assessment. These leaps—from rigid monitors to seamless, data-rich wearables—accelerate adoption in hospital-at-home and rehab, transforming passive tracking into proactive health management amid policy-backed digital health surges.
Aging population & growing healthcare expenditure

Remote patient monitoring (RPM)
Remote patient monitoring (RPM) presents a major opportunity for China's wearable medical device market by leveraging 5G infrastructure, aging demographics, and policy-backed digital health reforms to enable scalable chronic care outside hospitals, reducing readmissions and easing resource strains. Under the "Internet Plus Healthcare" framework and 14th Five-Year Plan, NMPA-approved wearables like Huawei Watch D2 integrate with telemedicine platforms for continuous vitals transmission BP, ECG, SpO2 to cloud dashboards, supporting follow-ups for hypertension and diabetes without offline prerequisites for stable cases.
By late 2024, over 4 million 5G base stations enabled low-latency RPM streaming, with pilots in smart wards using patches for ICU step-downs and home monitoring of 216 million elderly, aligning with the national target for 97% community-based senior care. For example, the Huawei Blood Pressure Health Study (2022–2024) across 31 provinces engaged 75,918 users, where 73.8% tracked BP via wearables, triggering 78% compliance on alerts and linking data to physician reviews, demonstrating RPM's role in averting escalations.
Internet hospitals, by June 2023, the number of internet hospitals had reached more than 3,000, and 364 million of China’s 1.079 billion internet users were using online medical services, with physicians needing 3+ years' experience and patient consent, fostering ecosystems where Xiaomi and Alibaba platforms aggregate wearable feeds for predictive analytics. Amid post-COVID hospital-at-home surges, RPM wearables address 300 million cardiovascular cases by enabling real-time interventions, positioning China as a leader in connected care.
Integration with AI and predictive analytics
Integration with AI and predictive analytics offers a significant opportunity for the China wearable medical device market by enhancing real-time diagnostics, personalized health management, and early disease detection, aligning with China’s strong policy backing and technological infrastructure. AI-powered wearables leverage advances in machine learning to interpret vast physiological data streams heart rate, ECG, blood pressure, glucose and generate predictive insights that support preventive care and early interventions, improving outcomes for chronic diseases like cardiovascular conditions and diabetes.
For example, tech conglomerates such as Tencent, Alibaba, and Huawei are deeply investing in AI-enabled wearable solutions; the partnership between Zhongshan Hospital, Huawei, and iFlytek developed multimodal AI tools integrating voice assistants and clinical decision support, while Tsinghua University’s AI system achieved over 93% diagnostic accuracy across 300+ diseases, illustrating how AI-driven wearables contribute to clinical decision-making and workflow efficiency.
The rise of 5G connectivity coupled with AI facilitates low-latency data transmission from wearables to cloud platforms, enabling continuous remote patient monitoring and predictive analytics within telehealth ecosystems, a synergy emphasized in government strategies like “Made in China 2025” and healthcare AI reform programs driving scenario-based adoption.

In May 2025, LifeQ partnered with BlueZone to launch a biometrics platform in mainland China, blending wearable data with AI insights for digital health apps focused on home-based wellness and remote coaching.
Market Factor Analysis
Regulatory Environment & Compliance
Regulatory Authority
The primary regulator is the National Medical Products Administration (NMPA).
There is also a technical evaluation body, the Chinese Medical Device Evaluation (CMDE), which plays a role in assessing registration dossiers.
Device Classification System
China classifies medical devices into three classes: Class I, II, and III, based on risk.
As Per the Provisions for Medical Device Registration and Filing (effective Oct 1, 2021), Class I devices are managed via filing, while Classes II and III require registration.
Recently (effective Sep 1, 2024), the NMPA updated its classification process for newly developed devices to better standardize classification submissions.
For wearable medical devices, many fall under Class II, due to their moderate risk; fewer are Class III.
Regulatory Requirements for Registration / Filing
Documentation: Applicants must submit a technical dossier (Product Technical Requirements, PTR) that includes functional/safety indicators, test methods, etc.
Quality Management System (QMS): The applicant/filer must implement a QMS appropriate to the product; for many devices, ISO 13485 is relevant, and Chinese GMP inspections may apply.
Labeling and Instructions: Instructions for use (IFU) and labels must be provided; submissions must be in Chinese (Simplified), and translated versions of foreign-language documents are required.
Standards: Devices must conform to applicable Chinese mandatory standards. If a device does not fully map to a standard (e.g., novel wearable), the applicant must provide a justification statement and supporting documentation.
Safety / Risk Management: Risk management principles must be embedded in R&D; every foreseeable risk should be managed, and benefits must outweigh risks under typical use.
Clinical Trials and Testing
Many Class II and Class III devices require type-testing in NMPA-designated labs in China.
For higher-risk devices (especially Class III), clinical trials may be required.
Importantly, trial requirements may demand in-country clinical evaluation or local data, increasing cost and complexity.
Registration Validity and Renewal
Registration certificates for Class II and Class III devices are valid for 5 years.
A renewal application should be submitted at least 6 months before expiration.
If there are significant changes in design, materials, manufacturing, or use, a “change-registration” is required; lesser changes may only need to be filed.
Priority & Emergency Procedures
Priority Registration: Some devices (e.g., those for rare diseases, or with significant clinical advantages) may be eligible for a priority registration path.
Data Privacy and Security Concerns
Data privacy and security concerns restrain the China wearable medical device market by eroding consumer trust, complicating regulatory compliance, and exposing users to breaches of sensitive health data under stringent laws like PIPL and Data Security Law, which classify medical information as highly sensitive requiring localized storage and security assessments for cross-border transfers. These regulations mandate explicit consent for processing, yet wearable firms often struggle with opaque policies; a 2025 systematic review of 17 manufacturers rated Huawei high-risk across 14 of 24 criteria, including 76% industry-wide high-risk for transparency reporting and 65% for vulnerability disclosure, with Asia-Pacific firms like Xiaomi (16 high-risk criteria) and Huawei showing elevated deficiencies in breach notification and user controls. For instance, wearable-collected biometrics, trajectories, and vitals risk tampering or deletion, endangering treatments leaked data could harm physical health or trigger psychological distress leading to worsened outcomes or even suicide in extreme cases.
Globally, breaches underscore vulnerabilities: a 2021 incident exposed 61 million fitness tracker records, while UnitedHealth's hack compromised 100 million health records, mirroring risks for China's ecosystem where devices generate trillions of daily data points amid third-party sharing. Surveys reveal 41% of reluctant users fear security breaches and 37% inaccurate data impacts, despite 87% favoring doctors using wearables, amplifying hesitancy in a market pushing continuous monitoring.
Regulatory Challenges and Approval Delays
Accuracy and reliability limitations restrain the China wearable medical device market by undermining clinical trust, delaying regulatory approvals like NMPA Class II/III certifications, and deterring physician adoption for diagnostic decisions, as consumer-grade sensors often falter under motion artifacts, skin variations, or diverse physiologies compared to gold-standard 12-lead ECG or cuff sphygmomanometers. A 2025 study on WenXinWuYang, an NMPA-approved portable single-lead AI-ECG device tested in 99 pregnant women (287 recordings), showed strong heart rate correlation (r=0.957) and QT interval validity (mean difference 2.1±12.0 ms, 88.9% strong agreement) versus 12-lead ECG, but QRS duration had low correlation (r=0.136) with 32.3% readings clustered at 89 ms due to algorithmic insensitivity, while PR interval agreement was moderate (58.6% strong, r=0.537), highlighting single-lead constraints in nuanced interval detection across trimesters.
Similarly, Huawei Watch D's Oscillo metric BP validation per ANSI/AAMI/ISO 81060-2:2018 in 85 adults yielded mean SBP/DBP differences of -0.25±5.00 mmHg and -1.33±6.31 mmHg (Criterion 2), passing standards, yet real-world factors like improper placement or user error cause inaccuracies, with studies noting up to 23.7% pressure decay at ±4 mm misalignment and degraded performance in heterogeneous groups (e.g., hypertensives, elderly).
Optical PPG-based trackers suffer motion-induced errors, yielding variable HRV or SpO2 readings, while a 2023 review flagged limited real-world evidence for arrhythmia specificity (sensitivity 84.2%, though specificity 97.5% for WenXinWuYang), risking false positives/negatives that could mislead treatments. These gaps, amplified by China's diverse demographics and strict post-market surveillance, slow mainstream integration despite validations.
Regulatory complexity
Regulatory complexity restrains the China wearable medical device market by imposing lengthy, multi-layered NMPA registration processes, clinical evaluation mandates, and evolving post-market surveillance that delay market entry, inflate compliance costs, and deter smaller innovators amid overlapping PIPL data rules and classification ambiguities for hybrid consumer-medical devices.
Wearables span Class II/III risks requiring technical reviews, biocompatibility tests in GLP labs per ISO standards, and quality system verifications, with domestic firms needing production filing post-registration while imports face local testing equivalents; the process demands detailed ISO 13485 design dossiers, product declarations under 2014 CFDA statutes, and up to 2–3 years for approvals. For instance, a September 2024 draft MDAL proposes eliminating home-country approval prerequisites for imports—previously mandating prior FDA/CE clearance allowing parallel filings, yet heightened penalties for high-risk violations like implants persist, alongside refurbished device bans.
In May 2025, NMPA expanded clinical trial exemptions to 1,047 devices (adding 27), but Clinical Evaluation Reports remain obligatory, complicating borderline wearables like AI-ECG patches. July 2025's Announcement No. 63 measures for high-end devices (e.g., AI wearables), including early expert consultations and phased inspections, yet intensifies post-market scrutiny via risk-based audits, real-world databases for cardiovascular implants, and inspector training pilots target surgical robots and 3D-printed sensors. These shifts under "Made in China 2025" foster innovation but burden firms with lifecycle obligations, from standards development to adverse event tracking, slowing adoption as foreign players navigate Domestic Responsible Persons and data localization.
Near-term growth will likely concentrate in modular bioreactor lines and closed-system media workflows that shorten validation cycles while preserving batch traceability.
Partnerships between CDMOs and instrumentation vendors should accelerate standard datasets for comparability across sites, improving forecasting models used in capacity planning.
Longer horizon, organoid and microphysiological adoption may reshape segment mix; teams that invest early in assay interoperability and cloud QC hooks are better positioned to capture upside without fragmenting their analytics stack.

Showing 105 of 105 companies
Codexis is a Redwood City-based enzyme engineering company in the middle of a deliberate transition. It is moving from a diversified "protein engineering" platform company, whose revenue was historically dominated by custom biocatalysts for small-molecule drug manufacturing, toward a focused provider of enzymatic manufacturing solutions for oligonucleotide therapeutics, chiefly siRNA/RNAi. Its proprietary CodeEvolver® directed-evolution platform underpins two businesses. The first is a cash-generating but lumpy pharma biocatalysis franchise, anchored by long-standing relationships with Merck & Co. and other large pharmaceutical companies, which supports about 14 commercial products. The second is the pre-commercial ECO Synthesis® Manufacturing Platform, which aims to replace solid-phase phosphoramidite chemistry with enzymatic routes, together with a commercially launched dsRNA ligase. The company is a sub-$200 million market capitalization micro-cap and remains loss-making. It has about $80 million of pro forma liquidity, a $40 million term loan, and guidance for FY2026 revenue of $72–76 million. --- ### Company's own description (FY2025 10-K / standard press-release boilerplate, paraphrased) Codexis describes itself as a leading provider of enzymatic solutions for efficient, scalable therapeutics manufacturing. It discovers, develops and improves high-performance enzymes using its proprietary CodeEvolver® technology. According to the company, its enzymes address real-world problems in small-molecule pharmaceutical manufacturing and nucleic acid synthesis. Codexis is developing the ECO Synthesis® Manufacturing Platform to enable scaled manufacture of RNAi therapeutics through an enzymatic route. It says its enzymes can raise yields, cut energy use and waste, improve manufacturing efficiency, and increase sensitivity in genomic and diagnostic applications (Codexis Q4 FY2025 and Q2 FY2026 press releases). The FY2025 10-K states that headquarters is both the administrative centre and the centre of research, development and business operations, with limited internal manufacturing capacity. In-house manufacturing produces Codex® biocatalyst panels and kits, plus enzymes for customers' pilot-scale and clinical production. The ECO Synthesis Innovation Lab sits inside headquarters and is designed to supply non-GMP material in quantities that support customers' preclinical development. The company expects to partner with CDMOs for bulk GMP-grade siRNA production for clinical supply and beyond. ### Independent characterization Codexis is best understood as a technology-licensing and specialty-enzyme supplier whose economics depend on a small number of very large pharmaceutical relationships. In FY2025 a single customer accounted for about 51% of total revenue (FY2025 10-K, "Customers"). Given the $37.8 million Merck agreement recognised mainly in Q4 FY2025, that customer is almost certainly Merck. The business model has three monetization layers. The first layer is product revenue: sales of engineered enzymes (biocatalysts), pharmaceutical intermediates, Codex® biocatalyst screening panels and kits, and, increasingly, ligase and ECO enzyme supply. Product revenue is driven by customers' commercial manufacturing schedules and is therefore volatile quarter to quarter. Product gross margin has improved structurally, from 56% in FY2024 to 64% in FY2025 and 71–73% in Q1–Q2 FY2026. Management attributes this to a shift toward higher-margin products and the run-off of lower-margin legacy products. The second layer is research and development revenue: CodeEvolver platform licence fees, technology-transfer payments, milestone payments, feasibility and evaluation fees, and ECO Synthesis manufacturing services (for example, the low-seven-figure contract to supply 50 g of siRNA). In FY2025 R&D revenue exceeded product revenue for the first time in the period analysed ($44.4 million vs. $26.0 million). The main reason was the Merck technology transfer. The third layer is royalty and contingent consideration from out-licensed or divested assets. These include the Nestlé Health Science CDX-7108 sale (up to $45 million in milestones plus a single-digit royalty), the Aldevron HiCap RNA polymerase licence (milestones and royalties), and the Roche dsDNA ligase licence (upfront and technical milestones). These are largely back-ended and uncertain. **Value-chain position.** Codexis sits upstream of drug-substance manufacturing. It supplies process technology (enzymes and the know-how to use them) to innovator pharma companies, generic manufacturers and CDMOs, which then run the commercial processes. In the oligonucleotide chain it is trying to move one step downstream. Its planned ECO GMP Manufacturing Center would make siRNA material for preclinical and Phase 1 supply. This hybrid "tool supplier plus early-phase manufacturer" model is a new risk profile for the company. **Customer types:** large innovator pharma (Merck, plus historically Pfizer, GSK and Novartis); high-growth RNAi biotech innovators (unnamed counterparties in 2025–2026 agreements); CDMOs (Bachem, Nitto Denko Avecia, Axolabs); and life-science tools companies (Roche, Aldevron, seqWell). **End-markets served:** small-molecule API manufacturing (biocatalysis); oligonucleotide/RNAi therapeutic manufacturing; and, as a residual licensing stream, life-science tools and genomics reagents. ### Revenue model mix (FY2021–FY2025) *Sources: Q4 FY2022 press release (FY2021–FY2022 PAXLOVID figures); FY2023 10-K (product revenue); Q4 FY2023 press release (ex-PAXLOVID revenue of $62.0 million implies about $8.1 million PAXLOVID-related revenue in FY2023, largely the release of deferred Pfizer fees); Q4 FY2025 8-K Exhibit 99.1 (FY2024–FY2025).
